Metaplastic breast carcinoma metastatic to the lung, liver and bone

Metaplastic carcinoma is uncommon and represents less than 5% of breast carcinomas have been reported. This type is a tumor characterized by a heterogeneous mixture of adenocarcinoma with other histological elements as squamous cell, spindle or another mesenchymal differentiation. We present a case of metaplastic breast carcinoma in an 63 year-old patient. An ultrasound (US) of the left breast revealed solid, heterogeneous lesion occupying the upper quadrants and the retroareolar region, measuring 15.1x8.6 cm. Ultrasonography of the left axilla showed ovoid homogeneous solid nodule with a diameter of 2.4x1.9 cm. The patient underwent modified radical mastectomy with axillary lymphadenectomy. The immunohistochemical analysis showed the diagnosis of invasive metaplastic carcinoma with squamous component and spindle cells. There was presence of distant metastasis to lungs, liver, left anterior rib. Currently, the patient is being treated with palliative chemotherapy of six cycles of 5-fluorouracil, epirubicin and cyclophosphamide.
