Primary breast sarcoma is rare, being leiomyosarcoma even less frequent, and in this group, the nippleareola primary leiomyosarcoma is extremely rare, with less than ten cases reported in the literature. In this paper, authors describe a case of a patient with a painful, fast growing lump located in retroareolar region of the right breast with a histopathological diagnosis of a yellowish white lobulated lump with a microscopic picture showing fusiform epithelioid cells with high proliferative activity, strong positive immunohistochemical results for smooth muscle actin, vimentin and desmin and negative immunohistochemical results for epithelial markers, hormone receptors and growth factor. Based on the morphological and anatomopathological findings, the diagnosis of Nipple-areola leiomyosarcoma was confirmed. A setorectomy procedure was performed which later results didn’t reveal any sign of residual neoplasia.
