Case report of a female patient who had an infiltrating ductal accessory breast carcinoma. The medical files, histological tissue and the scientific literature were reviewed. An out clinic 47-year old female patient was addressed to the Breast Surgery Department of Santa Casa de Misericórdia de Curitiba (Paraná) in 2006 to have a lower transition quadrant lump of the left breast observed in mammography. At the physical exam, an accessory breast with nipple-areola complex below the left breast was found, without palpable lumps or nipple discharge. Axillas and clavicular spaces were negative. A core-biopsy of the lesion revealed an infiltrating ductal carcinoma, moderately differenced, with positive hormonal receptors and negative CERB-B2. An accessory breast mastectomy with sentinel lymph node was done. The histopathological result revealed a infiltrating ductal carcinoma, moderated differenced with 1 x 0.7 cm, with no angiolymphatic invasion and two negative sentinel lymph nodes. Radiotherapy and tamoxifen were indicated for adjuvant treatment. Besides the rarity, the accessory breast cancer has high morbidity and mortality and deserves attention and caution.
